After booking please wait for the office to confirm the appointment with you. You can expect a call within one business day. Please fill out the paperwork beforehand, it will be sent to your phone via text and email after the appointment is confirmed. If you have a car accident/personal injury/workers’ comp case, do not fill out the paperwork sent to your phone, but if possible print and fill out the personal injury paperwork under the ‘intake forms’ tab on our website.
